Care in the Cloud provides a robust and adaptable end-to-end solution for a clinician working with a patient with a chronic condition. A shared action plan to set realistic behavior change goals and improve a patient’s health literacy and self-management skills is collaboratively created at the initial consultation. The shared action plan activates the patient and becomes a living document that evolves as the patient's needs change. Currently configured for a patient with diabetes, Care in the Cloud can be adapted to address other chronic disease states, care management or care transition workflow, according to customer specifications. Customers might also use our patented matching technology to tap Idle IQ™ (e.g., semi-retired or parenting health professionals) as part-time coaches for the benefit of patients in health professional shortage areas.
